Prosecutor: Requesting Restitution
ADULT CASES

To request restitution in all adult cases, you need to complete our
green Restitution Form and provide our office with supporting documentation of any out of pocket expenses, repairs or replacement of property. (This information will be requested in juvenile cases but at a later date by the probation department.)

Examples of supporting documentation are: estimates, receipts, evidencing payment of insurance deductibles, medical bills, co-pays, gas receipts for trips to medical providers or court appearances.  Please provide this to your Victim Assistance Specialist as soon as possible.

We recommend you give your information to your Victim Assistance Specialist within 15 days after you receive your first letter from our office however, we know in some instances this is not always possible.  In this case, please notify your Victim Assistance Specialist that you would like to request restitution for your crime-related out-of-pocket expenses, and get the documentation to him/her as soon as possible.  We must know about your wish to request restitution by the Pre-Trial Conference date otherwise it may not be ordered by the Court.

Please complete the Victim Rights Form and send to your Victim Assistance Specialist as soon as possible. 

Note:
If you have a personal injury due to the victimization, you may qualify for reimbursement with the Ohio Attorney Generalís Office - Victims of Crime Compensation Program.  For more information on eligibility requirements and to obtain an application form visit http://www.ohioattorneygeneral.gov/VictimsCompensation.aspx/?from=nav .

JUVENILE CASES

To request restitution in all juvenile cases, complete the
orange Victim Rights form and send it to your Victim Assistance Specialist.  Provide your restitution request on this form.  Then collect your supporting documentation for your request of your pocket expenses, repairs or replacement of property and once you are contacted by the Fairfield County Probation Department, provide their office with the needed supporting documentation.

We recommend you notify your Victim Assistance Specialist as soon as possible about your request.
